Medical Form to have More than 1 Custom Publishable Fields

I would like the Medical Form to have more than 1 custom publishable field. For example we have Please describe in detail any conditions checked if necessary and would like to also ask Has your Child ever had a Concussion? if so how many? which requires a answer of Yes/No and a Text Field. There is no other way to do this in the Form currently.

    Or enable the generic, unlimited "Form Field" object from non-medical School Forms. Might not be "publishable", but submitters can alway see what they submitted via Print of a form.

    There are Notes fields for Allergy, Condition and Medications. See Medical tab. They aren't enabled on Medical Forms though, which we've been pointing out for awhile. https://blackbaudk12.ideas.aha.io/ideas/K12OR-I-132
