We would like a list object added that will pull in the name of the inquiry form submitted

Our Admissions office wants to be able to pull a list or a report by date submitted and type of inquiry form, but currently the name of the form cannot be pulled into a list and the existing report for Inquiry Details, doesn't separate the information into separate columns when exported to Excel and is not easily sorted. This would be very important to us to be able to do as we try to pull this information for reporting purposes.
