In the form where it is entered, 'Preferred Hospital' appears under the 'Physician' heading, but is vauge: the Physician already has an address field, and if this is meant to be the Physician's Preferred Hospital, it should be labeled as that.  When treated as a Student's Preferred Hospital, this field needs much clearer labeling, as it appears (on the student's medical profile) without any label at all, just under the Physician's address.

Additionally, the field is not currently a column you can select in medical worklists, and does not export with the 'Physician' column selection.
