The way the medical form is designed, it pulls the list of allergies the nurse has added into the Medical module of the site. If we design a form where parents are required to specify what allergy their child has (if they selected that their child has one), if the allergy does not exist in the list the nurse added, there is no way for the parents to continue with the form. There needs to be an ability for a parent to select "Other Allergy" and then be able to type what the allergy is. The nurse would then get some sort of message letting them know of the allergy and then the nurse could choose whether to manually add that allergy to the list of allergies in the medical module.
