It would be extremely helpful to have the option to add at least one (if not more than one) custom question on the Medical Form that would then appear on the student's Medical Contact Card. We would like to be able to ask parents for permission to dispense certain over the counter medications (e.g. Tylenol) but cannot do so with the current form.
