Declined School Form should show under Declined only, not Started/Not Started

It is very confusing that the Declined forms show under Started and Not Started as well as the Declined category. We don't care if they declined before they started or not. It's possible other schools need to see that data, but it would be nice to have a toggle to turn off the duplicate entries.

    When parents decline a form it doesn't mark the form as complete which throws off the submission numbers. I would love if they counted declined forms as completed or submitted event when declined so it's clear that it is not an outstanding form that needs further review.
