The state of Ohio requires we collect the names and contact information for a student's preferred Physician, Dentist and Specialist (if necessary). The ability to use the Physician block more that once would be preferred. Even the ability to use a text box would be better than nothing. As it is now, there is only a space for one doctor and their information.
