When you print/view a completed form it looks completed different than the original form (what it looks like when users are filling it out). It's hard to read and visually confusing. Spacing changes and especially for multiple choice field forms you are not able to see what they chose very easily. My assumption is that only the option they choose is printed and the others are left off. This makes it very difficult to read these forms especially in a timely manner when it has to do with medical information.
