Medical Forms

In an effort to continue to move away from paper forms, we are looking to digitize our medical forms.

Currently we have parents fill out paper forms at the start of the year for their student health info (Asthma, Diabetes, Epilepsy etc. I am trying to create a digital version of these within the Academic>Content>School Forms.

As I am creating these forms I am trying to include as much information from the current templates we use (see attached). These are the forms I am trying to emulate.

I am finding I am limited within the existing fields I can edit or attach, what information can be included, I cannot edit certain drop-down fields that contain generic information. I would also like to have more free form type fields but there seems to be a limit on those.

My question is there a way to add more customization to the form field from the Blackbaud side of things? One of our administrators is looking for some information
