Custom Fields in Medical Module
I am looking for the Health module to be a location for Custom Fields to Publish to. Some fields that I need are:
· Care card number (the government health number)
· Medical alert checkbox with corresponding expiration date
· Physician practice name
· Insurance primary holder employer
· Dentist name
· Dentist phone

This would be a huge help - we have specific medical needs that we need to ask about that aren't addressed by the standard fields.
Absolutely!! The medical forms are too prescriptive to be useful.
If those fields could include uploads for signed physician forms (a yearly requirement) that'd be fabulous.
We need to have areas to document athletic consent exceptions.

We have the same issue, no way to collect and store the data in the Nurses Office section of the program.
The dentist field would be great
We need to ask specifically is we have permission to administer over the counter medications and need the field to be published in medical as well (not in the contact card where the publishable fields are).
