School Forms - conditional address fields

I would like School Forms to have Conditional Address Fields.

    Conditional address fields would allow a parent to enter a summer address - just active from June 1st - August 15th, for example.  Or a temporary address for a rental house while a new house is being completed with the dates the temporary address apply.
