It would be nice to be able to add a form field to a profile update form in school forms.

I would like to gather grandparent info using a profile update form, but I woud like to include a checkbox to see if parents would prefer that we do not contact the grandparents for certain things.

    Yes, please let there be tractions on this. It is cumbersome to gather data that would help us immensely when we have to create another Blank form just to make it easier for our parents.

    Yes - checkboxes on all profile forms would be fantastic.

    We currently use the Address Line 3 for collecting Yes/No responses on Profile Update forms. It's such bad data management - but since I can't put User Defined Fields in the profile update for anyone except students (Grandparents for example) - this is my work around. I then have to do multiple data refresh routines in order to remove the Yes/No from the Address field and get it into the User Defined Field.
