Self-Reschedule Include Original Date and Time

When a parent self re-schedules it currently only shows the updated date and time. It would be helpful to include what the original date and time were so Admissions can update lists.

    Great idea. It would also be helpful to see how many times someone re-schedules an appointment (perhaps giving insight into whether they might be a "high maintenance" family).
