Currently, we use "Blank Form" for a New Parent Questionnaire. Unfortunately, when it shows in Files & Forms it says "Blank Form" because the Type is listed. This confuses both parents and staff because the form has been completed but says "blank". The ability to overwrite or even HIDE the form type itself would be helpful.
