When someone besides the parent or candidate (ie. sibling, family member, guardian, agent) is submitting an inquiry, it's important to collect their contact info. There are fields for the inquiring person's name and relationship but contact info has to be collected by borrowing the fields from the candidate. This assigns incorrect information to the candidate during the inquiry processing stage.

Maybe this is a separate idea but there should be a much more prominent section on the inquiry form for the inquiring person and, coincidentally, a way to recognize that during inquiry processing.
