Official Notes are too important to a school to be left as unstructured as they currently are. In its current state, Official Notes makes teachers and administration vulnerable to errors that go directly to parents and students. There also isn't a thread of action plans or follow-up. The system needs the ability to allow a teacher to reply to previous Official Notes so that a teacher can track the communication and organize it together in a thread.

If not a thread, when a teacher replies there should be an embeded link to previous Official Notes for parents and administration to see the historical references.
