Applying a Style to a Giving Form Merged

I would like for there to be a way to apply a style to the Giving Form to make it look more visually appealing.

    Making all of our forms more visually appealing would be a plus. Our Admissions Application is very unprofessional and utilitarian looking. I'd like some spit and polish to be enabled for these documents.

    I would like to add the ability to make add conditions as well.

    For example, "if answered yes, open a text box to add details".
