I would like the ability to add all medical fields into Medical forms

We have 3 fields that have been added into Blackbaud to cover our provincial health care. I would like to be able to add these fields into medical forms. Without this functionality the fields have little value to us. These are not part of our 10 custom fields.

Canadian Health Card #, Card Expiry Date and From Province.

Agreed, medical forms are too restrictive given the different needs of individual schools - we need a pool of all options to choose from
