We just had a really bad accident at our school and when we pulled the printed consent form to give to the EMT's the child's birth date and parent contact or emergency contact information was not included.  This was the first thing the EMT's asked for.  This information needs to be included on this printable form.  Not just for emergencies such as this but for our coaches to take with them for away games, teachers for field trips.
