Ability for parents to upload school physicals, and for Nurse to track missing ones
Much like "Athletic Clearance", but allows parents to upload required school physical forms. Our state requires this of certain grade levels, so a mechanism to insert this into a required form is needed.
The current feature for parents to upload a physical is not enough. Our nurse MUST also be able to upload the form because so many parents still bring in a hard copy. How is this not an option? We also need a way to track when they expire that both the nurse and parents can see. I also agree that the nurse needs to be able to accept or decline (with a reason) the form uploaded. We have had several parents upload something that is completely unreadable or only one of the needed pages.
it is CRITICAL that both our Parents Nurses have an option to upload medical forms to a students medical record. Having a parent try to do it through School Forms has proven to be a nightmare. Please make a change before February 2022.
The file upload feature on the Medical school forms is great and addresses this, HOWEVER, it doesn't address the parents that can't or won't upload the form themselves. There is no mechanism where the school can upload the scanned paper on behalf of the parent.
This would be a fantastic feature. If they were to add it, I would add some way for the school to "Accept" the uploaded form which notifies the parents it has been accepted or "Deny" the uploaded form where the school can state why the form was denied. I collect 1300+ physical forms each year and many need to be returned to families due to problems on the forms.

Not having the ability to upload the actual physical paper form prevents us from using the Nurse's Office portion of Blackbaud.
