Permission Forms - More than one clickable option

We want to be able to create forms that allow a yes, I give permission or no, I do not give permission option before the parents signature for more than one customizable option on one form. 

 

For example:

Yes, I allow my child to walk home after school - No, I do not allow my child to walk home after school

Yes, I allow my child to drive home after school - No I do not allow my child to walk home after school

    We recently added the option to include Form Fields on permissions forms. These fields can be used to create Yes/no prompts in a school form.
