Medical form to collect COVID Vaccination/Booster cards are being submitted without complete cards. Fully vaxed and boosted cards are required by 3/27/22 but parents are submitting cards without boosters causing the nurse extra time/work to check the submissions/ allow resubmit if not completed and communicate with parents.
