Add Assigned Date for Consequence

Currently on onRecord, the dates range one creates for a consequence is the visible dates (ie, if you assign the consequence of demerit school and note the date range to be 11/2 - 11/5, demerit school is only visible to the student during that specified date range).  It would be useful for the consequence to always be visible and the date only be the assigned date (ie, When demerit school is assigned on 11/5, the parent and child can see that when they pull up the student's conduct record, so they know when the students must report to demerit school).

    Along the same lines, we would like to show the date range of a given Disciplinary Status (like Probation), but also have an end date for the status. We typically have a set time period or 3 months or 6 months for our Disciplinary Status, but we require students to follow up with each teacher to be removed from Disciplinary Status, and often this takes longer, so we would like an additional date field available to track the actual removal date, and all of these dates need to be visible to the student and parents, even outside of the date range!
