We have parents complete an online form called Dismissal Authorization Form.

The responses then go to report found on:

Core > Communication > Content Types > Custom Forms > Dismissal Form

I run it and then export it to excel and then post it on our email system for faculty to access.

I was wondering if it would be possible for what each family as entered to be posted onto their child's profile under Core as an additional category?

It would really help the nurse as she can see the Emergency Contacts and also the Authorized Dismissal.

It would be on this screen: (I used my son's profile as an example)
