I would appreciate the option to export information from the Medical Form to a csv. Many people in my office required printed versions of student data and it is not convenient to have to print 3-4 pages for every student. It would be better to just export information to a crystal report and run a one page report with allergy, medicine, medical history information.. Alternatively, you could create a one page medical summary report for Medical form data
@ Vincent Jurgens, never mind. just found it.
@ Vincent Jurgens, I cannot find the Medical Contact Cards "report" mentioned in your comment. Which roles have access to this? Thanks.
YES! We need the same functionality, but we also need to include Emergency Contact information. It is very frustrating for our front office to have to sort through several pages of forms for each student when trying to create our annual medical and emergency information binder (which we prepare in case of emergencies when online products are not accessible).
At the very least, it would be great to have this information accessible as an exportable, flattened, list so that we can create our own form for each student.
Thanks in advance.
Do you really mean Medical info core fields, not an OnRecord... Medical form type? The latter doesn't include Comment, Notes and other fields yet, so you may not want to focus rely on that. (We have lots of notes, comments entered via students Medical screens, that need printing.)
The new Medical Contact Cards "report", accessed only from Rosters, by certain roles, addresses this about 95%. It displays an html page, that when printed, puts one page per student. Also has emergency contacts and other useful things. (Maybe 1+ page, if student has tons of notes.)
Still doesn't have 100% of Medical fields, such as Allergy Reaction fields. But does always output fields you may not use, such as Permission to Treat which defaults to No for everyone. (We have a long legal custom School form for this.)
