Add phone and email fields to candidate connections

We often use our connections for relationships that the school would want to reach out to as part of the admissions process. We can put contact info in the candidate comment, but it would be better if it was actually categorized as contact info.

    To add, contact info should also be included for Community connections. Since these are pulled directly from database profiles, this should be as accessible as the name and grad year (all that shows currently)
