Candidate name on "Create an Account"
On the "Create an Account" (ex. https://ehs1.myschoolapp.com/app/#login/apply) we would like to change Candidate Name to Legal Name

I would like to be able to add fields. Specifically, we'd like to get Prefix for parent/Adult Account Holder.
