We use blank school forms when asking parents to give us arrival/departure dates and times. Sometimes their answers require non-linear time and/or the school to be open 365 days a year. Fields with date and time options would reduce errors and standardizing responses. This would be an advantage to add to any permissions form as well: parents sometimes need to communicate pickup/drop-off times and dates. (It would also be nice to be able to restrict responses, but that might be asking for too much.)
