Medical Form with Restrictive and Locked Fields Should Be More Flexible in Use

Currently, certain blocks in the Medical Form (namely Allergy and Conditions) have certain locked fields and fields that require a population of table values (allergies, conditions, medications, etc.). This is hard for a school to utilize as for:


1) Students all have a range of allergies, dietary restrictions, medications, etc. that you cannot account for. The blocks should have a space where non-listed items could be typed into a text field - maybe after choosing an "Other" option in the populated list of values.


2) Locked fields are unnecessary and do not work for all school situations. Our school in particular does not need to know when an allergy or condition started (As of Field) and "Who needs to see this" field. Actually, the latter field causes confusion because our institution's policy requires all residential personnel to have access to this information for emergency situations as we do not have a "nurse" on staff.


I would request that this is left up to the schools to choose the fields as they meet their needs.

    WE MUST LOSETHE "AS OF" DATE FIELD IN MEDICAL FORMS PLEASE!!! This field causes many issues each year and has absolutely no medical value. I also agree that the "Who needs to see this" should be optional and not a mandatory field.

    PLEASE FIX THIS!!!
