I tend to only use the Blank Form when creating forms for our school even though some of them are 'Acknowledgment of Risk' or 'Profile Updates' etc., because without the 'form field' the forms become very restrictive. With Acknowledgement form I generally want to add a 'i consent' field with a yes/no button.
In general, i find the pre-made forms to be difficult to customize. it would be much more useful if they were more flexible and customizable.
It's great to have a form field on all the forms, but if the information cannot be published to the user's profile it's still not super helpful. We desperately need to have addresses for emergency contacts, which would be an easy fix with the form field, except they need to be seen on the student's profile...
The form field is not showing up in Medical. We need it there, too!!!
On the Profile Update forms, I use the Address 3 (renamed) field on the grandparent address to have parents label if a grandparent is maternal or paternal so I know which one to connect through relationships. Then I have to remove all those entries out of the address field. A blank form field would be SOOOO helpful for this. It would also be helpful to use one so parents can let us know if a grandparent has died since the last update.
The fact that a "blank" form field cannot be added to all form types almost makes the school forms unusable. For example, the fact that a parent can't specify an allergy or a medication that is not on the prescribed list makes us (the school) look unprofessional.
Would be nice to have related blocks of information on a school form in addition to the 'form field'. For instance, address block, emergency contact block, and a form field to capture some yes/no consents such as photography policy, blanket field trip permission, etc.
The medical form also has no option to specify a 'other' allergy, condition or medication that is not listed. You can select 'other' but there is nowhere to put the information
Agreed - this is necessary form the re-enrollment contract as well
Also for Medical forms, there is no place to get a simple "needs epi-pen" or other custom field. This meant we had to use a blank form then upload our medical information.
