Please provide an Emergency Medical Information form Report or Export in a concise form for All (alpha sort) and by Grade that does not include all the empty form fields but only the information entered by parents. The need is to have a concise form of the information that is in the system for each student - hopefully not bigger that one two-sided sheet.
