Publish Nickname + Surname on Report card

Currently there is only the option to add the nickname at the end of a variety of options with the nickname in parentheses.  We populate our nickname with either the preferred name or the given name depending on what the student wants.  If we have no nickname we populate the given name.  The nickname field is never blank.  We would like to be able to produce report cards with Nickname + Surname.
