As another year rolls around, I once again long for this feature! Our poor parents have to refill their medical information from scratch every year because we can't use the Medical Forms without the Provincial Health Insurance Plan fields.

Please get this resolved. Once again, Canadian schools being ignored. It really makes life complicated for our nurses.

Once again, a huge disadvantage for Canadian schools not to have this included. Having the field available only from the admin end (Nurse's office) means if we want to use it, we have to do it manually. The whole point of medical forms is to give updating over to the parent side.

Agreed. Right now, we're using the regular "Policy Number" field on our medical forms, and changing it to say "OHIP Card". This causes problems with some families that have additional 3rd-party coverage, and also just makes the Provincial Health Insurance Field useless

It is really essential that this idea be implemented as soon as possible. Can this please be prioritized?

This idea was originally requested in 2016. Our organization has been waiting 2 years to see Provincial Health number appear on the medical form so that we can capture that required information. I believe Blackbaud should make this fix ASAP so it's Canadian customers have the same functionality as their US customers.

I AGREE. This form is very US centric. For my fellow Canadians I am using this workaround: I have added text that instructs local families to indicate "OHIP" as the Primary Healthcare Provider, their 10-digit health card number under Policy Number, and the two-letter version code under Group Number.

Really need this for start of school year

The Medical Form isn't very useful for Canadian schools if we can't ask for their Provincial Insurance details. We'll need to use a separate form as well and populate this field manually for each student. :(
