We use a fillable PDF for our teacher evaluation form, which we upload to the checklist via the "drag file here" box on the recommendation checklist step. When parents send an email to the teacher requesting he or she complete the form, the downloadable file is included in that email. Unfortunately, the blank file is also visible to the parents on the checklist. Please make this file visible only to the receiving teacher. When parents can see the file, it can increase anxiety and concern around the information that will be shared on the evaluation form.
