Adding a Parent Centric Inquiry Form

Oftentimes, we receive inquiries whereby the child's name is not known; and these inquiries cannot currently be added into OnBoard since the child's name is a required field on the inquiry form.  Not being able to enter these inquiries into the system skews any inquiry data we need to pull.  Please unlock any fields in the Sample Inquiry form, so we can create a form that is based on the parent information, not the child.

    If admin is filling out the inquiry form on the back end, all the content should be unlocked so its not required. It's ridiculous that we have to complete the exact same form that we publish if we are trying to add an inquiry in on the back end. If I am the person filling out an inquiry for a family, it's unlikely I know their child's birthdate for instance. Back end forms should not be the same as published forms.
