immunizations

spread sheet form or on a table, would be nice for immunizations, as there are several doses to complete different vaccinations series, example: Dtap requires 5 doses, we need to put in 5 dates, Hep B requires 3 doses, 3 dates, Polio requires 4 ect.

Also we need a place for a notation, example: Varicella vaccinations may not be required if the student had chickenpox and had titers drawn to prove immunity.  Another example if a student has a medical or religious exemption, we need to note that.  Therefore if there's an outbreak we can pull up student who have not been vacinated, if we just leave the immunizations blank, it looks as though we did not address immunizations for this child.

I completely agree! It is very time consuming to enter immunizations they way it is set up now - we shouldn't have to list each dose (Dtap 1, Dtap 2, Dtap 3, etc.) on the master list, then check the box AND enter a date for each one.

Our state requires certain immunizations to begin preschool, additional before Kinder and then a TDaP booster before 7th grade. There is no way to indicate that a particular immunization isn't due yet - the only choices are completed or missing. I'd like to be able to mark immunizations as due by ___ and then when that date arrives it should automatically update to indicate 'missing', or at least run a report to see which student has something due prior to today. Additionally, our state has some strange rules like 'if polio #3 is given after age 4, then a 4th dose isn't required' but I don't want to make dose 4 optional because it is usually required for compliance so there should be an option for 'not applicable' and a text box to enter a note about that.

I agree with all of these points. In the old Blackbaud product we used (Education Edge), there was a comment blank for each immunization on a student in which we could make comments on things such as exemptions from immunizations. We could also change the status of an immunization to exempt in each student's individual record.

Something else to consider is how hard it is to input dates for each immunization currently. We need to be able to input the date that each immunization was given because that indicates whether the state age requirements were met. Dates are not easy to input at all right now. It would make it easier if we could select the year, month, and date from the calendar rather than using the arrows to scroll from 2016 all the way back to 1999. Typing in the date is not terribly easy either in the current format.
