Notifications shouldn't go out to a given user more than once, even with multiple group membership

Users currently receive multiple notifications for an item when they belong to more than one group to which the notification pertains. Can we smarten the algorithm to send out the notification only once to a given user, even if they're in multiple groups?

    PLEEEEEEZ.  It is embarrassing when families get more than one of the same notification.
