Only 1/4 of the page is allotted to allergies, conditions and medications which are the CRITICAL portion of the medical card and can be extensive.The current format is NOT easy to read. I suggest combining all of the Medical Summary information with the Student Information and allowing 1/2 the page for allergies, conditions, and medications.

Our nurses keep paper copies in the office and an portrait version would also be great. In addition an option to include NOTES.

I understand that a list is planned for NOTES (thank you) but as they relate to the child, the nurse needs ALL information in one please. We want to use this form to replace our current paper copy.
