Include home address in emergency contact report

The emergency contact report currently includes medical data, parents numbers, emergency contact info, and physicians addresses but not their home addresses. If the student were unable to speak emergency personnel might need this info.

    We would love to see this, too. It would also be very helpful to have the student's legal name appear on the student's medical contact card.
