Designate a School Form as Medical, without being the 'Medical' School Form type

The 'Medical' School Form type is of limited use, and does not have the full flexibility of a 'Blank Form' School Form type (IE 'Form Fields' that are open-ended but do not map anywhere). Please let us designate an existing form as 'Medical' so that it can be HIPAA compliant.

    If I am reading this correctly, we would also be able to collect health information for our faculty/staff. The current medical form can only be assigned to the student or incoming student role.

    Specifically, our nurse would like to be able to use form fields and user signature (not just parent signature, we are a higher ed school)
